We recommend that you begin working on these items as soon as possible. They are due when you register for class. Failure to submit all the requirements may impact your eligibility for completion status of the Phlebotomy program. Click the button below to apply for training. Each program is designed and includes a “career readiness” component for all students to be successful. Upon graduating, each student is offered linkages to employment opportunity and resources.

Acceptable Documents

Here is a script that may help you with this process:
Would you please print off a copy of my Vaccine Administration Record / Immunization Summary/ Patient Vaccination Record of the above vaccines/ immunizations? It must include: the results, lot # if applicable (ex: Flu), my name, DOB, and dates of the immunizations. If you don’t have access to ALL of the above records, please draw my blood to check for immunity (Titer). For Flu, Covid and the 2 Step TB, I will need the shots. If I’m unable to have skin test done, you can draw for a Quantiferon. If TB results are positive, a Chest X-ray is required! Thank you! Questions: Call 513.309.4217
Below is a list of acceptable documents.

  • Physical Exam from a licensed medical provider. This helps us to address sitting, standing, and lifting.
  • Vaccinations - 2 Step TB, Hep B (all 3 doses or draw for immunity.
  • Flu (must be current)
  • COVID (fully vaccinated - Pfizer or Moderna. Booster not required)
  • Dtap or Tdap (only 1 is required, but it is okay if you have both)
  • MMR-Measles, Mumps, and Rubella (2 doses or draw immunity)
  • Varicella/Chickenpox (2 doses or draw for immunity.)
