Phlebotomy Technician Admission Requirements

**ALL STUDENTS MUST BE AT LEAST 18 YEARS OF AGE AND HAVE EITHER A HIGH SCHOOL DIPLOMA / GED. OR BE ENROLLED AS A HIGH SCHOOL SENIOR IN SECONDARY EDUCATION CENTER/SCHOOL**

A valid proof of identification must also be presented upon enrollment. Applicants must be able to speak, read, and understand English. All applicants will be asked to sign a “waiver of liability” and a “consent form” form upon enrollment. Students will not be allowed to begin class without signing this form.

Attendance/ Conduct Policy for Phlebotomy Technician​​


All students are expected to be dressed appropriately, no low cut shirts, short skirts or short shorts. No open toed shoes, sandals or flip flops are allowed to be worn.


Clinical Hours: Students must wear all black scrubs with a white lab coat.
